Ready to share?
The CIO asks one question: "What am I not seeing?"
1. Open Submit feedback.
2. Choose a mode. You choose every time:
Identified — your name is attached, so the CIO can follow up.
Anonymous — no name, email or account is stored with your feedback.
3. Add your feedback as typed text, one Word document, or one short video.
4. Optionally pick a category, then select Send.
Anonymous mode reduces what is recorded with your feedback. It does not guarantee that nobody can tell it was you.
Leave out patient names, medical record numbers, dates of birth, addresses, phone numbers, photos, and any other detail that could identify a patient. Describe the situation without naming who it happened to.
This app is not part of the medical record and is not a HIPAA-compliant system.
Included patient information by mistake? Contact the Privacy Office straight away. Submissions can’t be edited, but the CIO can delete one permanently.
