How this staff feedback channel handles what you send, and where its anonymity stops.
Last updated: October 27, 2026
This is a staff-only channel for Children's Nebraska IT. Only employees the CIO's office has added can sign in, and they answer one question: "What am I not seeing?" There is no public sign-up. Submissions are read by the CIO's office.
We collect the minimum needed to run the channel:
• Your account: name, work email, role, a hashed password (never the password itself) and your sign-in sessions.
• Your submission: what you type, and at most one Word document or one short video.
• An optional category you choose.
• If you use the mobile app and allow notifications: your device's notification token, which you can remove under Settings → My devices.
What you send is stored as you sent it. Nobody can edit a submission afterward, including the CIO's office.
You choose for each submission. Here is what is stored with it either way:
| Stored with the submission | Identified | Anonymous |
|---|---|---|
| Your name | Yes | No |
| Your email | Yes, through your account | No |
| Link to your account | Yes | No |
| IP address or browser details | No | No |
| When it arrived | Date and time | The day only |
| What you wrote or attached | Yes | Yes |
| In your own history | Yes | No — we can't tell it was yours |
Anonymous mode removes your name and account from the record. What you send can still point to you:
• Your writing style, or details only you or a few people would know.
• Your face or voice in a video.
• A Word document's own properties, which can carry the author's name. Remove them before attaching, or paste the text instead.
• Small-team context: on a team of a few people, the subject alone can narrow it down.
Our servers' general request logs, which are not linked to submissions, may record network details for security for a short time.
Do not include patient names, record numbers, dates of service or any other protected health information. This app is not designed or approved to hold PHI. If you need to raise a concern that involves a patient, use Children's Nebraska's established reporting channels instead.
• The CIO's office (the app's admins) can read every submission, tag it, add private notes and turn it into action items.
• Staff can send feedback and see their own identified submissions. Staff cannot read anyone else's feedback.
• The CIO's private notes are visible only to the CIO's office.
You can attach one Word document (.doc or .docx, up to 10 MB) or one video (up to 100 MB; a video recorded in the app stops on its own at the time limit). Files are stored privately and can only be opened by the CIO's office through short-lived links.
There is no automatic deletion schedule. Submissions and their attachments are kept until the CIO's office deletes them. Because an anonymous submission has no link to you, we cannot find or delete it for you on request. For an identified submission or your account, ask the CIO's office.
• We don't sell or share your information, or use it for advertising.
• We don't use your submissions for performance reviews outside the CIO's office.
• We don't try to work out who sent an anonymous submission.
