These terms apply to everyone who signs in to "What Am I Not Seeing?", the staff feedback channel to the CIO.
The app is for current hospital staff who have an account set up by the CIO's office. There is no public sign-up. Do not share your sign-in or let anyone else submit through your account. Your account is personal to you and can be deactivated when you leave.
The app answers one question: "What am I not seeing?" It is a channel for honest feedback to the CIO. It is not an emergency line, an incident-reporting system, an HR grievance process or a patient-safety report. For urgent safety concerns, use the hospital's established channels.
You choose identified or anonymous mode on every submission.
• Identified: your name and account are stored with the submission, with the date and time it arrived. You can see it in your own history.
• Anonymous: no name, email or account link is stored, and only the day it arrived. We can't tell it was yours, so it does not appear in your history.
Anonymous mode is not a guarantee. Your writing, details only you would know, your face or voice in a video, or a document's own properties can still point to you.
Limits of anonymity in the privacy policyDo not include patient names, record numbers, dates of service or any other protected health information (PHI). This app is not a HIPAA-compliant system and is not approved to hold PHI. If a concern involves a patient, use Children's Nebraska's established reporting channels instead.
Send honest, constructive feedback about how IT is led: what is working, what is not, and what the CIO may be missing. You can be direct. Keep it about decisions, processes and behavior rather than personal attacks.
A submission cannot be edited or taken back once it is sent, by you or by the CIO's office.
You may attach one Word document (.doc or .docx, up to 10 MB) or one video (up to 100 MB). A video recorded in the app stops on its own at the time limit. Only attach files you have the right to share, and remove anything that identifies a patient.
The CIO's office reads every submission. It may tag feedback against servant-leadership traits, keep private notes, turn it into action items and report on themes over time. Feedback is not used for anyone's performance review outside the CIO's office.
There is no automatic deletion schedule. Submissions and attachments are kept until the CIO's office deletes them. An anonymous submission has no link to you, so it cannot be found or deleted on your request. For an identified submission or your account, ask the CIO's office.
• Do not harass, threaten or discriminate against anyone.
• Do not submit content you know to be false, or that is meant to deceive.
• Do not upload malware, or try to get around the app's security or identify an anonymous sender.
• Follow Children's Nebraska's policies on acceptable use of IT systems.
The CIO's office may deactivate an account that breaks these terms.
We may update these terms. The date at the top of this page shows when they last changed. Continuing to use the app after a change means you accept the updated terms.
